If you are unable to come to your appointment, call or e-mail our office as soon as possible. This will allow people on the waitlist to obtain their reservation. Please extend this courtesy to others if you cannot make the appointment you booked.
All fees and appointment bookings are non refundable.
Appointments may be rescheduled with at least 24 hours’ notice. 
If you fail to show up for your appointment or arrive late by 5 minutes (or more), your appointment will be cancelled, and you will need to reschedule. Employer direct billed accounts will be charged a $57 missed appointment fee up to the cost of services ordered and fee to reschedule all missed appointments.
